We provide a coordinated support network that connects children and families to the medical, social, educational, and mental health services they need to thrive.
Personalized, Family-Centered Care
Our experienced and compassionate team serves families across Brooklyn and Staten Island. Dedicated Care Managers work closely with families and healthcare providers to ensure each child receives individualized, attentive care tailored to their unique needs.
We specialize in supporting children and youth with:
- Complex Childhood Trauma
- Serious Emotional Disturbance (SED)
- Multiple chronic conditions such as asthma, diabetes, and obesity
- Behavioral health challenges, including substance or alcohol use

How to Enroll
To participate in the Health Home Care Management Program, children must be enrolled in New York State Medicaid or Medicaid Managed Care.
